Acceptance U.S. passport applications in New Jersey: file Form DS-11 in person at a County Clerk acceptance facility
A County Clerk can accept a first-time U.S. passport application. The Department of State issues the passport. This site does not fill Form DS-11.

- First time, or not eligible to renew: apply in person on Form DS-11 at an acceptance facility. A County Clerk is one kind of facility. The clerk accepts the application; the Department of State issues the passport.
- Two fees. One fee goes to the Department of State with the application. A separate acceptance fee goes to the facility. Amounts and how to pay are on travel.state.gov, not on this page.
- Appointments. Many facilities require one. Ask the county page below, or use the facility locator, before you go.
- Official forms only on travel.state.gov. This site does not fill, store, or submit DS-11. Print the form from the State Department. Do not sign it until the acceptance agent tells you to.
